COVID-19 Molecular and Antigen Testing
Covid-19 and other respiratory pathogens, such as influenza viruses and respiratory syncytial virus, are typically diagnosed using antigen tests (RSV). Antigen tests that can identify SARS-CoV-2 have been granted emergency use authorization (EUA) by the Food and Drug Administration (FDA) of the United States. View the list of In Vitro Diagnostics EUAs compiled by the FDA.
Antigen tests are immunoassays that detect the presence of a particular viral antigen, which is indicative of an active viral infection. Currently, antigen tests may be performed on nasopharyngeal, nasal swab, or saliva specimens placed directly into the extraction buffer or reagent of the assay. There are currently authorized point-of-care, laboratory-based, and self-testing antigen tests. Certain tests have age restrictions; please visit the FDA's website for more information. Refer to Table 1 for more information regarding antigen tests.
The majority of antigen tests can be used at the point-of-care and produce results rapidly (within 15–30 minutes). Antigen tests for SARS-CoV-2 are typically less sensitive than RT-PCR and other nucleic acid amplification tests (NAATs), which detect and amplify the presence of viral nucleic acid. NAATs can detect levels of viral nucleic acid even when the virus cannot be cultured, indicating that the presence of viral nucleic acid does not necessarily indicate contagiousness.
